Description

Seniors at Risk in Hamilton collaborative is a partnership between Good Shepherd, Alzheimer Society and St. Matthew's House. Through this collaborative we provide support to seniors (55 years+) impacted by issues such as abuse/exploitation, mental and physical health challenges, dementia, precarious housing, food insecurity and social isolation. This support is provided to seniors individually or in a group setting. Referrals are received and processed by a centralized referral and intake process to facilitate timely interventions and supports. We also have telephone support provided by volunteers to isolated seniors and group programming (virtual and in-person) for Cantonese/Mandarin speaking seniors.
