Description
Complete the form for yourself, or someone you care for (family member, neighbour, friend, etc.) and someone from the Community Paramedicine Program will contact you.
The Community Paramedics are a non-emergency service that can perform in home medical and social health visits for:
- Getting connected to the right health/social provider to avoid escalation and crisis due to changing medical conditions/circumstances.
- Not eating or drinking as before, overall health is declining, worried about needing hospitalization, change in the “normal self”, or decline in looking after own self/home.
- General well-being check of blood pressure, heart rate, oxygen saturation, blood glucose, weight and/or temperature.
- Routine immunization such as a tetanus shot due to lack of a family doctor/nurse practitioner and/or inability to leave home.
If this is an emergency, please call 911 or go to your nearest emergency room.

Eligibility Criteria:
- 55+ years
- Client is on an long term care (LTC) waitlist, is eligible or soon to be eligible for an LTC waitlist.
