Description

Palliative care is delivered by a multidisciplinary team called the Palliative Care Community Team, currently including a Registered Nurse (Patient Navigator) and Social Worker. When a diagnosis of a life-limiting illness has been received, the Patient Navigator will work with other members of the Team, Community Programs, and the Central East Home and Community Care to ensure that clients/families are receiving all the services available to provide the highest quality of life up to, and including, end of life care. Palliative Care can be provided on an inpatient basis at HHHS - Acute Care if required. Patient supports available: • Family Physician access - 24 hours a day/7 days a week (through nursing agency) • Volunteer visiting • Coordinated support to Health Services • Access to pain and symptom management • Client-centered inter-professional Case Coordination
