Located in the “Feel Good” town of Cobourg*, our program provides personal supports for older adults who need higher levels of assistance to live as independently as possible. Visits are scheduled based on need during the day, evening, or overnight, with on-call support available 24/7.

Eligible recipients who qualify for Assisted Living/Supportive Housing are identified through assessment by Ontario Health at Home. To qualify, individuals must be able to transfer independently or with minimal support.

The Assisted Living/Supportive Housing program is publicly funded, so there is no cost to you. However, fees apply for congregate care setting accommodations and other related services.

Common tasks may include:

• Personal Care: Assisting with bathing, dressing, grooming, prompting medications, wellness checks, and toileting.
• Homemaking: Light housekeeping and meal preparation.
• Care Coordination: Assistance managing care and accessing other support services.
