Description

Request a phone assessment from a Community Paramedic for yourself, or someone you care for (family member, neighbour, friend, etc) to determine if a Community Paramedic can offer an in-home visit. The Community Paramedics are a non-emergency service . Their team is a highly skilled group that can perform in home medical and social health visits for reasons such as: - Getting connected to the right health/social provider to avoid escalation and crisis due to changing medical conditions/circumstances. - Check of blood pressure, heart rate, oxygen saturation, blood glucose, weight and/or temperature for individuals with chronic diseases that are not managed and resulting in hospital admission. - Routine immunization as requested for homebound individuals. If this is an emergency, please call 911 or go to your nearest emergency room.
