Eligibility Criteria
- 55+ years
- Services are provided within the client's home to adults 55 years of age or older who need very limited services to remain living independently in the community. Must have some degree of functional impairment related to a long-term chronic condition or frailty and need assistance with ADL's in order to restore or maintain their independence.
