Description
In collaboration with existing services, Community Paramedics act as an extension of primary care and serve the needs of the Renfrew County community. Home visits are conducted by advanced care paramedics throughout the County of Renfrew. Paramedics complete thorough assessments, vital signs, point of care testing, medication administration, vaccinations and blood work. All of the information from each home visit is then relayed to the patient's primary care provider.
Point of Care Ultrasound (POCUS) is used to assess for presence of various pathologies. This is not a comprehensive (formal) ultrasound examination.

Eligibility Criteria:
- The patient meets one or more of the following criteria: unattached (no Primary Care Provider), house/bed-bound, isolated or living in a remote area, multiple co-morbidities, eligible for LTC, aged 75+, Post Stroke, Post hospital discharge, CPRPM, Palliative Approach to Care. If your patient meets other criteria, select "No/Unsure" and enter a reason for review.
- The patient has given consent to submit this referral.
